Park Ave Villa – Assisted Living Facility:

NA/CNA/MA

This position provides tenants with routine cares, medication assistance, housekeeping and various tasks assigned as established by Park Avenue Villa Assisted Living procedures and as directed by supervision. This is a part-time position for 20 hours per week, generally evening shifts (either 2:00 PM to 10:00 PM or 4:00 PM to 10:00 PM) with weekend/holiday duty rotation. Ideal candidates will have experience as a NA, CNA, or MA, but training available. Applicant must be in good standing with ND Nurse Aide Registry.

St. Luke’s Home – Long Term Care Facility:

Case Manager Admissions Coordinator

This position reports to the Director of Nursing and is responsible for ensuring medically, emotionally, and socially related needs of residents are met and maintained on an individual basis in accordance with policies, procedures and federal, state, and local standards and regulations. Works directly with consulting medical facilities for admission and discharge planning. Manages delivery of quality resident care by ensuring care plan compliance. Requires skill in planning and organizing nursing care. Applies advanced nursing clinical skills in maintaining records and charts. Prepares written reports; works directly with social services, admission, discharge, nursing team, therapy, mental health, and hospice referrals. Must possess, at a minimum, a Nursing Degree from an accredited college or university; RN licensure/experience preferred.

LPN or RN Charge Nurse

This position directs nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants, certified medical assistants and support personnel. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services, to ensure that the highest degree of quality care is always maintained. Mainly part-time evening and overnight shifts available. Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Certified Nursing Assistant (CNA)

This position provides quality nursing care to residents; implements specific procedures and programs; coordinates work within the department, as well as with other departments; reports pertinent information to the immediate supervisor; responds to inquiries or requests for information; and assists the immediate supervisor with tasks to support department operations. There are full-time and part-time positions available, with alternating weekends; must have a current CNA certification and be in good standing with the ND Nurse Aide Registry.

Certified Nursing Assistant (CNA) – Geriatric Mental Health Unit

This position provides quality nursing care to the geriatric population of mental health; demonstrates specific procedures and unit operations; coordinates work within the department, as well as with other departments; reports pertinent information to the immediate supervisor; responds to residents mental and physical activity of daily living needs; and assists the immediate supervisor with tasks to support department operations. Full-time and part-time positions available with alternating weekends; must have a current CNA certification and be in good standing with the ND Nurse Aide Registry.

Dietary Assistant

In keeping with St. Luke’s Home/PAV tradition, we seek to enhance the quality of life and holistic growth of residents by providing high quality service in a cheerful, timely manner and by communicating, coordinating, and cooperating with others to attain goals. The Dietary Assistant is responsible for providing food service to residents, guests, and employees under sanitary conditions and for aiding in all dietary functions as directed and in accordance with established dietary policies and procedures. This ad is for a full-time, day shift position with alternating weekends. Interest in part-time Cook duties preferred.
