Park Avenue Villa – Assisted Living Facility:

NA/CNA/MA I

This position provides tenants with routine cares, medication assistance, housekeeping and various tasks assigned as established by Park Avenue Villa Assisted Living procedures and as directed by supervision. This is a part-time position for 16-24 hours per week, generally evenings and overnights.

Ideal candidates will have either NA, CNA or MA I experience; applicant must be in good standing with ND Nurse Aide Registry.

St. Luke’s Home – Long Term Care Facility: 

MDS Coordinator

MDS Coordinator gathers information on the facility’s current residents for future assessment, including physical and mental states. MDS coordinator assesses charts and communicates with health care teams to create applicable health care plans for their current and incoming residents. MDS coordinator is responsible to coordinate the shipment of their facility’s services to its residing residents. They also observe and document the pricing and effectiveness of these services.

The MDS nurse must conduct new resident assessments, such as the Rapid Assessment Process (RAPS), to help stabilize and improve the practices of their health care facility. They also provide OBRA (Omnibus Reconciliation Act of 1987) assessments to ensure that the facility complies with Medicaid or Medicare standards. An MDS coordinator may be required act as the main communicator between the insurance professionals and management officials at their facility

RN, LPN or MA II for Med Pass

St. Luke’s Home is excited to offer positions to qualified RN, LPN, or MA II personnel who are interested in 4-hour shifts and assisting with medications. Hours of shifts are 6 AM to 10 AM and/or 5 PM to 9 PM. Job duties include passing medications, diabetic monitoring, and treatments. Individuals will be able to work under their scope of practice and facility policies. These individuals must maintain either a ND RN license, LPN license, or MA II qualification in good standing by the accrediting authorities. Full time and part time positions are available.

Certified Nursing Assistant (CNA)

This position provides quality nursing care to residents; implements specific procedures and programs; coordinates work within the department, as well as with other departments; reports pertinent information to the immediate supervisor; responds to inquiries or requests for information; and assists the immediate supervisor with tasks to support department operations. Must have a current CNA certification and be in good standing with the Nurse Aide Registry.

Certified Nursing Assistant (CNA) – Geriatric Mental Health Unit

This position provides quality nursing care to the geriatric population of mental health; demonstrates specific procedures and unit operations; coordinates work within the department, as well as with other departments; reports pertinent information to the immediate supervisor; responds to residents mental and physical activity of daily living needs; and assists the immediate supervisor with tasks to support department operations. Full-time and part-time positions available. Must have a current CNA certification and be in good standing with the ND Nurse Aide Registry.

Activity Assistant – Geriatric Mental Health Unit

This position assists the activity director in performing his or her duties within the activity department. Depending on the area an activity assistant works in, the duties and responsibilities may differ. The general duties include:
• Encourage participants to make choices for themselves
• Present programs in front of a group of participants
• Encourage social exchange with staff and other participants
• Assist in various small group activities
• Assist the activity director in creating and planning activities
• Participate in meetings
• Respond to management direction
• Reorder equipment or supplies used for activities and plans
• Ensure that any equipment is functional and other related activities

Applicants must possess a valid state-issued driver license. CNA certification is required due to the nature of the duties. This is an evening shift position with either all or some hours working on a geriatric mental health unit.
